Make a Referral

Referrals can be made by Care Coordinators, Social Workers, Occupational Therapists, Resettlement Managers, Commissioners or Social Services

Step 1

You or someone acting on your behalf should contact your local authority to request a social care assessment. This is to establish your eligibility to receive care under the 2014 Care Act. The local authority will then carry out a financial assessment to work out funding

Step 2

Complete the electronic referral form below.To speed up the process, please complete as much details as possible with all the mandatory spaces completed. This is usually carried out by a social worker or a representative within the local authority.

Step 3

If we agree that supported living is the best next step, a detailed free assessment process follows. All people who form the individuals support network; health and social work professionals, care coordinators and family will be asked to attend along with the referred individual.

Step 4

We will liaise with the local authority about our assessment and wait approval for funding. Once approved, we will agree a visit and move in date to your chosen accommodation and begin drafting an individualised care plan.
